This video demonstrates image-guided endoscopic removal of a penetrating nasal foreign body and skull base repair in a 39-year-old status post nailgun injury. Preoperative imaging showed a 3-inch metallic foreign body traversing the left nasal cavity with violation of the anterior skull base. Using a right transsphenoidal approach, the foreign body was removed under direct visualization. Skull base was repaired with DuraGen inlay and free mucosal graft onlay from the middle turbinate.
