P379: MICROSCOPIC AND ENDOSCOPIC COMBINED RESECTION OF RECURRENT LEFT CAVERNOUS SINUS MENINGIOMA

We report a 72 year old woman with recurrent left cavernous sinus meningioma causing diplopia and trigeminal numbness after previous sphenoid meningioma resection and radiotherapy. Preoperative ICA balloon test occlusion and embolization of the left middle meningeal and accessory middle meningeal arteries reduced tumor vascularity. Microsurgical resection using a left orbitozygomatic craniotomy and middle fossa approach was combined with endoscopic endonasal transpterygoid removal of residual skull base tumor, achieving gross total resection and symptomatic improvement.
