Adenoid cystic carcinoma (ACC) of the head and neck is a rare salivary gland malignancy notable for perineural invasion, recurrence, and metastasis. We present a case of recurrent ACC of the anterior skull base requiring multidisciplinary intervention. Endoscopic skull base access, orbitofrontal craniotomy with optic nerve decompression, and vascularized flap reconstruction were performed collaboratively by neurosurgery, otolaryngology, and oculoplastics. This case highlights the technical challenges and importance of comprehensive team-based surgical management in advanced ACC.
