A 67-year-old woman presented with confusion, cognitive decline, and unsteady gait. Brain imaging showed a large olfactory groove meningioma and an incidental anterior communicating artery (AComA) aneurysm immediately posterior to it. Concomitant microsurgical tumor resection and aneurysm clip ligation were successfully performed using a bifrontal interhemispheric approach. Postoperative imaging demonstrated gross total tumor resection and complete aneurysm obliteration. The patient’s mental and cognitive functions improved significantly and she regained her ability to ambulate independently.
