Here we show the case of a 37-year-old male who presented to the ED with complaints of neck pain, hoarseness, and dysphagia. Imaging demonstrated a large lytic skull base mass involving the clivus and petrous apex. Biopsy demonstrated a chordoma. Resection was performed via a transnasal and transpalatal approach. Residual tumor was noted which was removed during a second stage and reconstruction was achieved with an anterolateral thigh free flap.
