We describe the surgical technique and highlight the relevant anatomy for treatment of geniculate neuralgia in a 41-year-old female using microvascular decompression and sectioning of nervus intermedius. Using a retrosigmoid craniotomy, nervus intermedius was identified as an independent fascicle located between seventh and eighth cranial nerves. Following nerve identification – and confirmation with neurostimulation – the nerve was successfully sectioned using microscissors. Postoperatively, the patient experienced complete symptom resolution with no complications at three-month follow-up.
