A 75-year-old woman with bitemporal vision loss and normal pituitary function was found to have a solid-cystic suprasellar mass, suspicious for papillary craniopharyngioma given lack of calcifications on imaging. She underwent an endoscopic endonasal biopsy and decompression via a transsellar transtubercular approach. Pathology confirmed the diagnosis, so surgery was stopped. Postoperatively, her pituitary function remained intact. Given the tumor’s BRAF mutation sensitivity, targeted BRAF/MEK inhibitor therapy is reserved for recurrence, avoiding the need for radiation.
