P269: EXTENDED ENDOSCOPIC ENDONASAL APPROACH FOR RESECTION OF PARASELLAR/SUPRASELLAR EPIDERMOID CYST

A 62-year-old woman with progressive vision loss was found to have a large cystic suprasellar mass compressing the optic apparatus. Differential included craniopharyngioma, Rathke’s cleft cyst, and epidermoid cyst. An extended endoscopic endonasal approach achieved complete resection. Pathology confirmed an epidermoid cyst with keratin debris and squamous epithelium. Suprasellar epidermoid cysts are rare, comprising <5% of cases. This case highlights the feasibility and novelty of extended endoscopic endonasal resection for safe management of such lesions.
