A 38 year old woman with multiple left cranial neuropathies and left ear mass was found to have multifocal paraganglioma syndrome caused by SDHD mutation. She was found to have a very large left jugular lesion, bilateral carotid lesions, and two right vagal lesions. All were DOTATATE positive. Herein we present the workup and simultaneous resection of the left carotid and jugular lesions.
