P080: EXTENDED RETROSIGMOID APPROACH IN THE SITTING POSITION FOR RESECTION OF A PETROCLIVAL MENINGIOMA

This video demonstrates the resection of a large petroclival meningioma via an extended retrosigmoid approach in the sitting position. The tumor compressed the brainstem, effaced the fourth ventricle, and extended into Meckel’s cave and the internal auditory canal. A gravity-assisted, piecemeal resection was performed with bimanual dissection and preservation of brainstem perforators. Postoperative MRI confirmed gross total resection with minimal residual at the IAC and complete brainstem decompression. The patient had no neurological deficits.
