This is a rare case presentation of symptomatic intracranial hypertension from compression of the right internal jugular vein by the transverse process of C1. The patient underwent an extreme far lateral approach for the resection of the transverse process of C1 to decompress her intracranial venous outflow. Special surgical consideration was made for the preservation of the superficial veins that contributed to anastomotic venous flow, as well as the jugular vein and the vertebral artery.
