P043: ENDOSCOPIC RESECTION OF A CAVERNOUS AND CLINOIDAL SPACE-INVASIVE PROLACTINOMA

We present a 51-year-old female with a prolactin-secreting pituitary adenoma, refractory to cabergoline, with invasion of the cavernous and clinoidal spaces. She underwent endoscopic endonasal resection, including wide sphenoidotomy, exposure of the clinoidal carotid, and stepwise opening of the medial cavernous sinus wall with preservation of the pituitary and carotid artery. The tumor was successfully removed from the cavernous, clinoidal, and sellar compartments. This case highlights technical nuances for safe, maximal resection of invasive prolactinomas.
