Meningioma growth during pregnancy is a well-documented problem which presents many medical and surgical issues. This case is a 36-year-old female G7P6 presenting with 2 months of progressive visual loss with OD 20/600 found to have a large tuberculum meningioma eccentric to the right invading the optic canal. An endoscopic, endonasal, transtuberculum, transplanum approach was selected to resect her tumor. Simpson grade 1 resection was achieved, and her post-operative vision improved to OD 20/40.
