Operative approaches to the pineal region and posterior incisural space are difficult due to long, awkward working trajectories and sensitive surrounding neurovascular structures. The objective of this video is to illustrate the surgical treatment of a vermian hemangioblastoma facilitated by the parietooccipital interhemispheric precuneal transtentorial approach. This video highlights considerations for positioning and approach selection for patients with lesions in the pineal region and posterior incisural space.
