Case presentation of a 52M with vertigo due to compression of the brainstem and cranial nerves by the vertebral artery and PICA. Treatment consisted of a far lateral craniotomy and decompression via mobilization of the vertebral artery and suturing to the clival dura as well as microvascular decompression with muscle and teflon. In our series of 18 patients, vertebral arteriopexy resulted in resolution of symptoms without new neurologic deficits in the majority of patients.
