We present the case of a 40-year-old male who presents with right upper and lower extremity weakness and numbness. He was found to have a large, calcified intradural, extramedullary lesion at C1-2 with arterial encasement of the right PICA. The patient underwent a suboccipital craniectomy and C1-2 laminectomies, with gross total resection as well as patency of PICA confirmed with intraoperative ICG. The patient was discharged home with improved motor function.
