Here we present a surgical video of an endoscopic resection of a pineal region tumor in a 46-year-old male with a remote history of previously treated pineal germinoma. The patient underwent resection and high-dose radiation in 1999. Recurrence was identified in 2024, and gross total resection was achieved. The patient’s pathology was consistent with a sarcoma.
