P281: INTRODUCTION OF A NOVEL TECHNIQUE FOR MUCOSAL SUTURING OF THE NASAL SEPTUM IN ENDOSCOPIC ENDONASAL SURGERY: A STEP-BY-STEP DESCRIPTION

BACKGROUND: Along with expanding the boundaries of the extended endonasal endoscopic approach, the risks of morbidity and damage to surrounding structures can also increase. An effective and watertight closure is mandatory not only in cases of intradural tumors or cerebrospinal fluid (CSF) leaks, but also as a barrier between the nasal cavity and the exposed neurovascular structures
OBJECTIVES: To present the improvement of the endonasal suturing technique. To assess the morbidity associated with the procedure, comparing the indications, advantages, disadvantages, limitations, and complications of the technique.
METHODS: The technique was applied to a total of 27 patients who underwent clival and paraclival surgeries using a mononostril chopstick targeted clival approach.
RESULTS: We described, with illustrative cases in addition to clinical and epidemiological data, a new technique for performing endonasal suturing in targeted approaches to clival lesions.
CONCLUSIONS: Details of the technique are provided, as well as its potential limitations. The rate of infectious complications or CSF leak in our series was similar to that found in the literature with other closure techniques. This technique was also associated with a low rate of nasal morbidity.
