11th Annual Meeting

Skull Base Surgery 2000 - General Information

MEETING LOCATION All scientific and social events for the 2000 Annual Meeting of the North American Skull Base Society (NASBS) will be held at the DoubleTree Paradise Valley Resort in Scottsdale, Arizona, unless otherwise indicated. The Pre-Meeting Courses, the hands-on Workshop and Practical Course will be held at the Barrow Neurological Institute in Phoenix, Arizona. Transportation will be provided between the resort and the BNI.

REGISTRATION The enclosed registration form should be completed and mailed to the NASBS. All meeting participants MUST register for the meeting. The deadline for the early registration fee is JANUARY 3, 2000. The deadline for advance registration is FEBRUARY 25, 2000. After this date, all changes or additions may be made on-site at the meeting.

ACCOMPANYING PERSON REGISTRATION Spouses or guests of registrants who do not attend the scientific sessions, may register as an Accompanying Person. The registration fee includes a ticket to the social events which include the Welcome Reception, the Wine & Cheese Reception and the Annual Banquet.

CANCELLATION AND REFUND Cancellation and refund requests must be submitted in writing. If cancellation for the Annual Meeting is received by FEBRUARY 25, 2000, the registration fee will be refunded with a deduction of 10% to cover administrative expenses. After FEBRUARY 25, 2000, no refunds are possible. See the next page for additional information for the cancellation and refund policy pertaining to the Pre-Meeting Courses.

HOTEL RESERVATION The DoubleTree Paradise Valley Resort is located in Scottsdale, Arizona, approximately 20 minutes from the Phoenix Sky Harbor International Airport. A block of rooms with special room rates of $219 single or double occupancy has been set aside for meeting attendees. A housing reservation form is included in this brochure. The deadline for hotel reservation is FEBRUARY 14, 2000. These special rates are available to meeting attendees two days prior to and/or after the meeting in case you need to extend your stay.

AIRLINE DISCOUNTS United Airlines is the official airline of the North American Skull Base Society Annual Meeting. if you or your travel agent call United's toll-free number (1-800-521-4041) to book your reservations, you will receive a 5% discount off the lowest applicable discount fare, including First Class, or a 10% discount off mid-week coach fares, purchased 7 days in advance. An additional 5% discount will apply when tickets are purchased at least 60 days in advance of your travel date. Discounts also apply on Shuttle by United and United Express. Call United at 1-800-521-4041 and reference Meeting ID Code 555HW. Dedicated reservationists are on duty seven days a week from 7:00 am to 12:00 midnight EST.

RENTAL CARS DISCOUNTS Alamo Rent A Car has been selected as the official car rental company. To receive the special group rates, meeting attendees must call Alamo's toll-free number (1-800-732-3232) and reference Group ID number 606957, rate code: GR. SOCIAL ACTIVITIES Plan now to attend the Welcome Reception on Thursday evening, March 16. This will be a wonderful opportunity for you to greet old friends and to meet new ones. On Friday evening, March 17, attend the Wine & Cheese Reception in the Exhibit Hall. This would an opportune time to visit all the exhibits and to validate your entry form for the Exhibit Hall Grand Prize drawing.

The Annual Banquet is scheduled for Sunday evening, March 19, western cook-out style. We have received word from the Local Hospitality Committee that no ties will be permitted. Additional warnings will be posted.
