Skull Base Surgery 2000 - Call for Abstracts

INSTRUCTIONS
The deadline for receipt of abstract is NOVEMBER 1, 1999. A special form is not required. List the presenting author's full name (including middle initial and degree) and complete mailing address (E-MAIL ADDRESS OR FAX NUMBER IS MANDATORY) as well as names of co- authors. All correspondence regarding the abstract will be sent only to the presenting author. Print abstract in the space provided. Abstract should be approximately 150 words in English. Indicate author's presentation preference. The presenting author should also complete the Disclosure of Commercial Relations section of the abstract form. If the abstract is e-mailed, receipt will be acknowledged within 24 hours. All abstracts will receive a mailed acknowledgement. The presenting author will be asked to proof-read and confirm the abstract title and author information. As soon as the Program Committee has selected the submitted abstracts for presentation, the presenting author will be notified by e-mail or fax. Please note that all presenting authors must be registered for the Annual Meeting.

ABSTRACT CONTENT
1. The title should be in upper and lower case letters along and inside the top of the box space in which the abstract is typed. If possible, the title should be in bold type. Preferred font is 12 point Times Roman. Author information should not listed here, but listed in the section of this form requesting presenting and co-author information.
2. Adequately describe the objectives of the study so that the quality of the work can be evaluated by the Program Committee.
3. The abstract should contain an introduction presenting the purpose of the study, a brief description of the procedures employed, a summary of the results and a statement of the conclusions.
4. The abstract should be approximately 150 words in English.

SUBMIT YOUR ABSTRACT
The Program Committee STRONGLY REQUESTS THAT ABSTRACTS BE SUBMITTED BY E-MAIL. The abstract and all the information on the form may be e-mailed as an attachment (Word or Word Perfect format, any version), or pasted into an e-mail message as generic text. If the abstract contains tables or complicated symbols, it should be sent as an attachment. To submit the abstract by e-mail, present the information requested in this order:

  • Presenting author address & co-author information
  • Title of abstract
  • Abstract
  • Presentation preference
  • Answers to disclosure of commercial relationship questions
